Civil War Pension Documents
You can order these from the National Archives and Records Administration.
First you have to send for the order forms - they arrive within a week. Then you fill them
out with whatever information you've gathered. You can get a small file, with items of
genealogical interest (up to 10 pages) for $15, or the complete file for $37. Then you wait.
It can sometimes take nearly a year before the records arrive. If they don't find any
records, they don't charge you.

pension documents for him.

It appears from the Rolls on file in this Office, that Thomas J Baker was enrolled on the
13th day of Aug, 1862 at Boonville, Ind. in Co. "K", 12th Regiment of Ky. Ca'v'y
Volunteers, to serve ~~~ years or during the war, and mustered into service as Q.M. Sgt
on the 17th day of Nov 1862, at Owensboro, Ky in Co K, 12th Regiment of Ky. Ca'v'y to
serve 3 years or during the war. On the Muster Roll of Co K of that Regiment (stuff
crossed out) from Enrollt. Dec 31st, 1862, he is Dropped. On roll of Co. "M" for the same
time he is borne among the Co. officers without rank being stated. on 4 Mo's roll Q.M. (?)
to Apr. 30/63, Pvt. Present. On 4 Mo's roll to Feb'y 29/64, Pvt. Present. (No evidence of
disability) No further record of him on said Co. On roll of Co."H" for Mc. & Apr./64 said
name is borne as present without remark. Was honorably Must'd out of that Co. as Pvt.
Aug. 23/65. No evidence of disability.

To all whom it may concern: know ye, that Johnathan B Lusk a Corporal of Captain Albert
Knowles Company, (D), 120 Regiment of Indiana Infantry Volunteers who was enrolled on
the twenty first day of February on thousand eight hundred and sixty four to serve three
years or during the war, is hereby DISCHARGED from the service of the United States
this eighth day of February, 1866 at Raleigh, North Carolina by reason of SO no 2 dated
Hdqrs Dept NC Jan 2d 1866 (No objection to his being re=enlisted is known to exist.*)
[note - this sentence will be erased should there be anything in the conduct or physical
condition of the soldier rendering him unfit for the army.]
Said Johnathan B Lusk was born in Greenup County in the state of Kentucky, is thirty one
years of age, five feet eleven inches high, florid complexion, gray eyes, dark hair, and by
occupation, when enrolled, a farmer.
Given at Raleigh NC this eighth day of January 1866.

years or duration of war, mustered in as private 8 Mar 1864 at Vincennes. On the muster
rolls ... he is reported present on all rolls and m/o with his company at Raleigh NC Jany
8/66 a corporal.

Affidavit - Warrick Co - 7 May 1873 before AJ Honeycutt clerk of circuit court
age 52 - private in Co E, 120 Ind Inf Vols. - discharged as a private at Louisville 22 July
1865 -
Atlanta - mid-July 1864 - neuralgia in left jaw -> unfit for duty - treated in field hospital,
moved with regiment for about a month. Disease proved to be Necrosis of the lower
jawbone -> Joe Hold Hospital at Jeffersonville, Indiana - treated ~4mos. then to general
hospital at Jeffersonville until day of discharge (~2weeks). Never recovered - bone had to
be taken out by Dr. JW Wertz of Jasper, Duboys County, Ind.

personal description: age 52, height 5'11 ½ ", hair dark, eyes blue, complexion dark.

Absalom trying to get invalid pension. first taken with winter fever - Nashville TN 5 Apr
1864 - treated at a general hospital there, then transferred to Jeffersonville hospital and then
furloughed home. Then returned to hospital ~August - Dr. wanted him to be invalided out,
but he rejoined his regiment just about 5 days before reg. left Atlanta to chase Hood -
about which time he contracted the disease with which he is now suffering. He followed his
regiment, sometimes marching, sometimes in an ambulance until he reached Nashville just
before the fight with Hood. About 10 days after the fight with Hood he was sent back to
Jeffersonville hospital.
